What documents can I expect to receive?

You will receive (i) a ballot paper (ii) Form 2 (iii) an Envelope A (iv) and Envelope B.
If you vote for Parliamentary and also State, you will receive two Form2 and two Ballot Paper. One set of Form2 + Ballot Paper for Parliamentary, one set of Form2 + Ballot Paper for State.
Form 2 is a declaration of identity that you are the voter who received the ballot paper.

- Please ensure that all information in Form 2 is correct including the Ballot serial number. The Ballot serial number should correspond with the number stated in Form 2.
- Form 2 must be signed (by you) in the presence of a witness.
- Witness must be a Malaysian aged 18 and above.
